Our new patient portal is a convenient system that allows patients on-line access to medical records, appointments, billing and more. The program is available in both English and Spanish and allows patients to:Compose Messages, Request Medication Refills, Request Appointments, Request Health Record, View Statements and access the Research Center. 

 

If you do not have a Patient Portal Login and Username, see a staff member and request a Patient Portal Policy and Procedure packet at one of the CQMA locations in Bisbee,Douglas or Palominas.
